Role Summary
The LPN plays an autonomous and supportive coordination role in a Collaborative Care Model providing holistic, resident focused care to meet the defined needs of individuals and families. The LPN develops therapeutic and professional relationships with Residents and members of the Health Care Team respectively for providing safe, effective, and ethical care to a defined group of residents with stable and predictable outcomes.
The LPN utilizes foundational nursing knowledge, evidence, critical thinking, and judgment to provide safe, competent, compassionate, ethical nursing care on both an independent and collaborative basis respecting the LPN scope of practice.
